LA EPISTEMOLOGÍA NO PRODUCE CIENCIA

La epistemología da cuenta de cómo se produjo una ciencia, no produce ciencia. La epistemología materialista aporta los conceptos epistemológicos con los que se puede leer cómo se produjo la teoría del valor en la economía política, la teoría del falo en el psicoanálisis o la teoría general de la relatividad en la física actual.

Después desde cada teoría se desprende una ideología, que se transmite inconscientemente, en la que el sujeto de la ciencia puede pretender explicar cualquier fenómeno bajo el prisma de los conceptos. Eso ya no es la teoría eso es lo Absoluto que reina en todos los corazones, el espacio absoluto, el tiempo absoluto, el dios absoluto, la omnipotencia del sujeto fulano de tal.

Rescatemos algunos conceptos epistemológicos, como instrumentos de lectura para la producción de salud, entre los que destacamos: Concepto de Trabajo, Concepto de Tiempo y Sobredeterminación.

Concepto de TRABAJO. El trabajo tiene como efecto la producción de algo nuevo que no existe previamente. Con este concepto de trabajo, el sujeto psíquico es una producción o una construcción que acontece en análisis. ¿Un niño nace con salud o se produce con trabajo? Nacemos indefensos, carentes y por el deseo de otros crecemos, después deseamos deseos y se produce el desear.

Los factores que intervienen en el proceso de trabajo son: materia prima, instrumentos y efectos en la realidad. Podemos decir que enfermar conlleva un trabajo y mantenerse enfermo también requiere un trabajo en el proceso de enfermar. En este proceso de trabajo está incluido: un proyecto, una puesta en acto y un pensamiento concreto. Ejemplo: el paciente habla libremente (contenido manifiesto) eso es materia prima y sobre ella actúan los instrumentos de la escucha y el saber inconsciente del psicoanalista y se produce la interpretación como efecto de ese trabajo. Con las diferentes interpretaciones se construye una historia de deseos inconscientes. Si ahora partimos del deseo inconsciente podemos hacer otro tipo de trabajo y reconstruimos los mecanismos y operadores psíquicos que han intervenido en la producción de ese deseo.

Concepto de TIEMPO. Hay un concepto de tiempo Real, aristotélico que es cronológico, lineal, es el tiempo como transcurso del pasado hacia el futuro, es el tiempo del calendario, igual a sí mismo, mensurable. Es el tiempo del diagnóstico, pronóstico y tratamiento de la medicina. El tiempo que maneja el psicoanálisis es un tiempo recurrente, viene del futuro, es discontinúo, es el tiempo del inconsciente que se produce desde la interpretación, es el tiempo Histórico del sujeto y ahí el tratamiento psicoanalítico hace al diagnóstico.

El proceso de enfermar entonces no está dirigido por el pasado, por lo que pasó, por lo vivido, sino por lo que digo, por lo que hago, por la posición del sujeto en el lenguaje, por la implicación futura en el proceso de enfermar o la producción de salud. Así lo referido por el paciente, los síntomas, lo fantaseado, lo que se le atribuye a la enfermedad es contenido manifiesto para el psicoanálisis. Es materia prima para trabajar lo nuevo.

Según se concibe el tratamiento se conciben los criterios de salud y de enfermedad.

Concepto de SOBREDETERMINACIÓN. El psicoanálisis es una ciencia de efectos, no de causas. Partimos de lo manifiesto no como causa de nada, sino como materia prima para ser trabajada con instrumentos precisos y se produce autoconocimiento y autotransformación según los deseos del paciente.

Cuando el paciente habla, manifiesta un montón de cosas que están múltiplemente determinadas, desde su conciencia por los modelos ideológicos de la educación, la familia, el barrio, el estado, la religión, las ciencias, los medios de difusión, la moral…Toda esa múltiple determinación que habla en el sujeto está sobredeterminada desde lo Inconsciente. No elije al azar un color, un número, no cuenta un sueño exactamente igual si le invitamos a repetirlo o no se afecta una víscera por casualidad, hay una historia significante en cada dolencia.

La Sobredeterminación nos dice que un efecto está sobredeterminado cuando es producto de una estructura compleja. Decimos compleja porque en dicha articulación los conceptos mantienen relaciones invariantes con otros conceptos, dicho de otro modo, la sobredeterminación es inconsciente. Y el sujeto de todo el material preconsciente (múltiplemente determinado) elige de manera significante.

En el ejemplo de un sueño, cuando el paciente nos cuenta un sueño, el analista escucha lo significante, no prejuzga, ni aconseja, ni contradice, escucha en atención libremente distribuida, escucha lo significante que representa al sujeto para otro significante. Y no se trata de devolver al paciente cómo vivió, sino cómo desea. No hay una causa única ni se puede agotar lo inconsciente, ni preexiste antes de ser interpretado. Las causas se construyen después de ser interpretadas.

Otro concepto es la CAUSALIDAD PSÍQUICA. La causalidad psíquica la pensamos desde la función de la IMAGO, es decir desde la IDENTIFICACIÓN inconsciente que se produce en todos los sujetos. La Identificación es la apropiación de la imagen del semejante. En un primer tiempo me identifico a la especie, al ser (me identifico con otro ser vivo para no ser confundido y así el hombre se cree hombre). La imagen produce al sujeto y así se construye un cuerpo en la llamada Fase del espejo (6 a 15 meses de vida).

A los seis meses el niño puede mantenerse sentado y al año da los primeros pasos en brazos del lenguaje. Posteriormente hay un segundo tiempo en el que nos apropiamos de las diferencias sexuales y de los 3 a los 5 años transita el Complejo de Edipo como centro de la operación simbólica de Castración. Posteriormente hay un tiempo de Latencia (de amnesia) que desemboca en la Metamorfosis de la Pubertad. Y desde ahí se articulan los conceptos fundamentales del sujeto.

Y si nos preguntamos ¿cómo representar el cuerpo y la mente?, el soma y el psiquismo se pueden representar topológicamente desde la Banda de Moebius. No son lo mismo y no son lo uno sin lo otro. El cuerpo y el psiquismo se constituyen gracias al mecanismo inconsciente de la Identificación. No siendo la realidad exterior lo que nos determina, sino la realidad psíquica la que sobredetermina. Y aquellas primeras relaciones de la sexualidad infantil edípicas que quedaron reprimidas para siempre, viven y laten en lo inconsciente de forma permanente y se manifiestan a través de los diferentes efectos: lapsus, olvidos, sueños, síntomas, chistes…Esto resulta inexplicable desde la razón, desde la conciencia y el Yo del sujeto lo que no entiende tiende a racionalizarlo, es decir hacer conocido lo novedoso.

En cualquier proceso de enfermar ya sea psíquico, orgánico y/o psicosomático, siempre intervienen mecanismos psíquicos. Sabemos que también participan factores fisiológicos y sustancias químicas que forman parte de la múltiple determinación del soma. Y los mecanismos psíquicos que intervienen son los mismos mecanismos para la producción de salud que para la producción de la enfermedad, tanto para la mujer como para el hombre, en oriente y en occidente, sea pobre o rico.

En el proceso de enfermar lo que sobredetermina son las Leyes del Lenguaje, es decir las leyes del funcionamiento psíquico sobre las leyes del funcionamiento fisiológico y bioquímico.

Según lea la realidad desde una estructura u otra, según los instrumentos de lectura que se utilizan o según sean los criterios de salud y enfermedad se obtienen diferentes miradas, pronósticos, tratamientos y diagnósticos sobre el proceso de enfermar. Ejemplo: En la medicina la causa es el cuerpo, se encuentra en el soma el agente etiológico. Para el psicoanálisis la causa es el lenguaje y estamos enfermos por hablar, somos carentes por proceder de seres sexuales.

La medicina es la ciencia de las enfermedades. El psicoanálisis la ciencia de la salud.

El sujeto biológico enferma, muere cuando el sujeto psíquico deja de desear. Y todos deseamos las mismas cosas, nos diferenciamos en las distintas maneras de renuncia.

 

CAPITULO SIETE: PSICOANÁLISIS DE LA SALUD. CRITERIOS DE SALUD Y ENFERMEDAD

EPISTEMOLOGY DOES NOT PRODUCE SCIENCE

Epistemology accounts for how a science was produced, it does not produce science. Materialist epistemology provides the epistemological concepts to read how the theory of value was produced in political economy, the theory of the phallus in psychoanalysis or the general theory of relativity in current physics.

After each theory emerges an ideology, which is transmitted unconsciously, in which the subject of science can try to explain any phenomenon under the prism of concepts. That is not the theory that is the Absolute that reigns in all hearts, absolute space, absolute time, the absolute God, the omnipotence of the so-and-so subject.

Let us rescue some epistemological concepts, as reading instruments for the production of health, among which we highlight: Concept of Work, Concept of Time and Over determination.

WORK concept. Work has the effect of producing something new that does not exist previously. With this concept of work, the psychic subject is a production or a construction that occurs in analysis. Is a child born with health or in the other hand is produced with work? We are born defenceless, lacking and by the desire of others we grow, after, we desire desires and desire occurs.

The factors that intervene in the work process are: raw material, instruments and effects in reality. We can say that getting sick involves work and staying sick also requires work in the process of getting sick. Included in this work process are: a project, an implementation and a concrete thought. Example: the patient speaks freely (manifest content) that is raw material and the instruments of listening and the psychoanalyst's unconscious knowledge act on it and interpretation is produced as an effect of that work. With the different interpretations a history of unconscious desires is built. If we now start from the unconscious desire we can do another type of work and reconstruct the psychic mechanisms and operators that have intervened in the production of that desire.

Concept of TIME. There is a Real time concept, Aristotelian time that is chronological, linear, it is time as the passage from the past to the future, it is the time of the calendar, equal to itself, measurable. It is the time of diagnosis, prognosis and treatment of medicine. The time that psychoanalysis handles is a recurring time, it comes from the future, it is discontinuous, it is the time of the unconscious that is produced from the interpretation, it is the historical time of the subject and there the psychoanalytic treatment makes the diagnosis.

Therefore the process of becoming ill is not aimed by the past, by what happened, by what is lived, but by what I say, what I do, the position of the subject in language, by the future involvement in the process of becoming ill or the production of health. Thus what is referred to by the patient, the symptoms, the fantasized, what is attributed to the disease is manifest content for psychoanalysis. It is raw material to work the new.

Like the treatment is conceived, the criteria of health and illness are conceived as well.

The concept of OVERDETERMINATION. Psychoanalysis is a science of effects, not causes. We start from the manifest not as the cause of anything, but as a raw material to be worked with precise instruments and self-knowledge and self-transformation occurs according to the wishes of the patient.

When the patient speaks, he manifests a lot of things that are multiply determined, from his conscience by the ideological models of education, family, neighbourhood, the state, religion, science, the media, morality ... all that multiple determination that speaks in the subject is overdetermined from the Unconscious. He does not randomly choose a colour, a number, he does not tells a dream exactly the same if we invite him to repeat it or a viscera is not affected by chance, there is a significant story in each ailment.

Overdetermination tells us that an effect is overdetermined when it is the product of a complex structure. We say complex because in this articulation the concepts maintain invariant relationships with other concepts, in other words, overdetermination is unconscious. And the subject of all preconscious material (multiply determined) chooses in a notable way.

In the example of a dream, when the patient tells us about a dream, the analyst listens the signifier, does not prejudge, advice, or contradict, the analyst listens in freely distributed attention, he listens to the signifier that represents the subject for another signifier. And it is not about returning to the patient how he or she lived, but how he wishes. There is no single cause, nor can the unconscious be exhausted, nor does it pre-exist before being interpreted. Causes of it are constructed after being interpreted.

Another concept is PSYCHIC CAUSALITY. We think of psychic causality from the function of the IMAGO, that is, from the unconscious IDENTIFICATION that occurs in all subjects. Identification is the appropriation of the image of the fellow man. At first I identify myself with the species, with the being (I identify with another living being in order to others do not be confused and so human believes to be human). The image produces the subject and thus a body is built in the so-called Mirror Phase (6 to 15 months of life).

At six months the child can remain seated and at one year the child takes the first steps in the arms of language. Later there is a second time in which we appropriate the sexual differences and from 3 to 5 years the Oedipus Complex passes as the centre of the symbolic Castration operation. Later there is a time of Latency (of amnesia) that leads to the Metamorphosis of Puberty. And from there the fundamental concepts of the subject are articulated.

If we ask; how do we represent body and mind? The soma and the psyche can be represented topologically from the Moebius Strip. They are not the same and they are not one without the other. The body and the psyche are constituted thanks to the unconscious mechanism of Identification. It is not external reality that determines us, but psychic reality that over-determines. And those first Oedipal infantile sexual relationships that were repressed forever, live and beat in the unconscious permanently and are manifested through different effects: lapses, forgetfulness, dreams, symptoms, jokes ... This is inexplicable from reason and consciousness and what the ego of the subject does not understand tends to rationalize it, that is, to make known what is new.

In any process of becoming ill either psychic, organic and / or psychosomatic, psychic mechanisms always intervene. We know also it is involved physiological factors and chemical substances that are part of the multiple determination of soma. And the psychic mechanisms that intervene are the same mechanisms for the production of health as for the production of disease, both for women and men, in East and West, poor or rich.

In the process of becoming ill, what is overdetermined are the Laws of Language, that is, the laws of psychic functioning over the laws of physiological and biochemical functioning.

Depending on how reality is read from one structure or another, depending on the reading instruments used or depending on the criteria of health and disease, different views, prognoses, treatments and diagnoses are obtained about the process of becoming ill. Example: In medicine the cause is the body, the etiological agent is found in the soma. For psychoanalysis, the cause is language and we are sick because we speak, we are lacking because we proceed from sexual beings.

Medicine is the science of disease and Psychoanalysis the science of health.

The biological subject becomes ill, dies when the psychic subject stops desiring. And we all desire the same things; we differ in the different ways of renunciation.
